• Download the form then fill it up with the desired course & necessary details.

• Attach the essential documents:

  • Self-attested copy of Marksheets & Degree Certificates.

  • Character Certificate.

  • Three recent photographs of the candidate.

  • Self-attested photocopy of ID Proof (Aadhar Card, Voter ID Card etc.)

  • Cast Certificate (for candidates belonging to Reserved category, SC/ ST/ OBC.)

  • An Affidavit (in case the candidate has a gap of one or more year) to justify that the candidate was not involved in any questionable activities.

  • Candidate with minimum 40% disability must submit a certificate issued by the Chief Medical Officer to be eligible for admission.
